You'll need a State Id. Driver license. Or Passport
I'f a bank ask you to enter the FuLL SS# after it FIRST require the last four _ _. _. _.

Then that' means you just raised a flag, either there is some inconsistency with the SS# , the information entered , or that profile has fraud protection

You'll need to have a clean and unaltered profile; (pros) in fraudster's terms. This will require a Social Security number, credit report, and TruthFinder background and ID number. Once you've got those, head over to the bank's website and click 'Open new bank account'. Fill out all the details from your pro exactly as given. When you submit it will ask for some security questions; refer back to your TruthFinder report for those answers, and make sure you get them right. That's pretty much it - just follow the steps correctly and it shouldn't be too hard!
